Overview

Options for Long Term Care (OLTC) provides access to publicly funded programs, providing support to people with disabling functional conditions and limited finances. These programs are designed to help people continue to live in their own homes or in other community settings, as an alternative to nursing home care.

 

OLTC provides assessment and ongoing case management to older and disabled low income individuals who are in need of long term care services in their own homes or alternative care facilities. OLTC also provides assessments for people in need of placement in a skilled facility. OLTC currently provides ongoing services to approximately 1300 Larimer County residents outside of nursing facilities, with the goal of providing long term care in the settings requested by individuals  and their families and utilizing Medicaid in the most cost effective manner.

Details

 Specifically this position:

  • Manages a caseload of approximately 100 + individuals on an HCBS waiver and or complete initial assessments for individuals applying for Long Term Care Medicaid programs
  • Provides crisis intervention as needed to assist individuals in decision making and problem resolution; acts as advocate for the individual
  • Assists individuals in determining services and supports to meet their needs in the community in an effort to prevent nursing home placement.  Assesses for NF placement when appropriate.
  • Coordinates services including scope and duration while ensuring timeliness of service delivery.
  • Ability to create a service allocation based upon functional need.  
  • Educates individuals regarding Medicaid benefits, programs and non-Medicaid services in the community.
  • Monitors individuals quarterly or more often to ensure the ongoing provision of need and satisfaction of services
  • Ability to embrace change and be flexible
  • Works cooperatively with internal  Department of Human Service agencies including but not limited to LTC Medicaid Tech’s, Adult Protection and Office on Aging.
  • Ability to work in various settings including but not limited to: Open office work environment, individual’s home and in the community.

CM 1 Qualifications:

  • Bachelor's Degree from an accredited college or university with major coursework in Sociology, Psychology, Human Services, Social Work, Public Administration, or a related field. Applicants who will receive their degree within one semester/quarter will be considered for this position. 
  • Possession of, or ability to obtain, a valid Colorado Driver's License.
  • Complete training and/or certification as mandated by the assigned program.
  • An offer of employment is contingent upon the successful completion of a pre-employment drug screen and background check. As marijuana is an illegal substance under federal law, testing positive for marijuana (medical or otherwise) or any other substance for which the county tests, will result in the contingent offer of employment being revoked. A successful completion of the drug screen is defined as obtaining a negative result.

CM II Qualifications:

  • Same as qualifications listed above for CM I plus one year of experience in a directly related field or in the performance of similar duties and responsibilities.

This position is limited term with full time hours.  Individuals hired into limited term positions are eligible for County benefits, however, the position is limited due to contract or agreement with the State of Colorado.
